Bulk webpage outline fetcher

Fetches heading outlines for up to 20 pages at once.
It runs the webpage outline fetch on up to 20 URLs in one call, processed concurrently, and returns results in the same order you sent them, each with a per-item error field. It gives the same answer per URL as the single-URL page outline endpoint. It reports a count of failures alongside the results, for batch enrichment when you already hold a list of URLs.
